Merrol Hyde磁铁学校

Welcome to Merrol Hyde 体育运动 - Educating the Whole Child

The 不仅如此 philosophy of educating the whole child—mind, body, and spirit—is one of the core values at MHMS.  要做到这一点, and to promote a positive culture of spirit and pride within the student body, all students in grades 7-12 are required to participate in a school sport each year.  Students must attend all practices, games, and/or performances in order to fulfill this requirement.  As you think about the sport(s) you would like to play, keep these things in mind: transportation to away games and off-site facilities is not provided by the school; and 8th graders are allowed to participate in some high school sports.  Students enrolling at MHMS after a team has conducted tryouts will be offered a supplemental tryout.

If you participate in a sport not offered by MHMS, you may request to substitute that sport to fulfill the MHMS sports requirement.  替换 MUST be approved before the season starts and meet certain guidelines.  Substitution forms may be requested from 代克雷格黑德 via email.

If you feel you should be exempted from the sports requirement due to a health reason, 请电邮先生. 弗兰克在 This email address is being protected from spambots. You need JavaScript enabled to view it..  Health information will be kept confidential.

Each student-athlete must have a current sports physical each year in order to participate in an official school sport.  体检日期是4月15日, 2022 or after will cover the student for all of the 2022-2023 school year.  All official school sports will use Final Forms to upload their signed Physical Examination and Medical Eligibility Form, as well as sign all necessary digital forms. Look at the list of sports below to see which require you to get a physical and use Final Forms.  Please reach out to 代克雷格黑德 with any questions.

The following are official Merrol Hyde School sports and must use Final Forms to complete all physical steps: MS和HS越野, 高中女生排球, HS高尔夫, HS女子足球, MS and HS Girls and Boys Basketball, HS保龄球, MS和HS啦啦队, 跳舞, HS男子足球, HS女孩和男孩网球, MS和HS轨道.

The following are Merrol Hyde activities that must still use Final Forms, but they do not require an actual doctor's physical.  However, we still strongly encourage your child to get a physical and upload it to Final Forms: 射箭与个人健身

All other sports offered at Merrol Hyde are "club sports" and do not require you to submit a physical to school through Final Forms.  The coach will share any paperwork with you that is needed.  You can see the full list of offered sports under the "athletics" tab.

Print this and fill out the history portion before going to your physical appointment: Preparticipation History and Physical Examination/Medical Eligibility Form

Create a Final Forms account using the instructions here: 最后形式家长剧本.  Once you create your parent account, you'll complete and sign all necessary digital forms for your child's sport and upload the Physical Examination Form (with doctor's signature) and Medical Eligibility Form (with doctor's signature). Scan or take a quality picture of both pages to upload to the "TSSAA PPE Physical" tab on your child's Final Forms account.
